Job summary

AuthoraCare Collective in North Carolina seeks a PRN Speech-Language Pathologist Case Manager to support our Home Health department serving Alamance and Rockingham Counties. The SLP will provide speech-language therapy per the patient’s plan of care, assessing cognition, swallowing, and communication, and tailoring home exercise programs for recovery.

You will work with an interprofessional team, document progress, and educate patients and caregivers.

Qualifications

• Master’s or doctoral degree in speech-language pathology.
• Minimum of one-year speech therapy experience in home health or facility preferred.
• Current and unrestricted Speech Therapy Licensure.
• Current CPR certification.

Responsibilities

• Provide speech-language therapy per patient plan of care.
• Assess cognition, swallowing, speech, and develop home exercise plans.
• Collaborate with IDT to manage caseload and establish treatment plans.
• Document goals and outcomes of care; support hospice as needed.
