Job summary

Amcare Pro Home Health, Palliative Care & Hospice in Fort Worth seeks a compassionate Physical Therapist for a PRN field role delivering in-home therapy across the Fort Worth area. You will assess patients, create and adjust treatment plans, and coordinate care with the interdisciplinary team.

The role emphasizes mobility, independence, fall prevention, safety in the home, and documentation in EMR. Texas license and BLS are required; travel is expected as you support patients in their homes.

Qualifications

• Current Texas Physical Therapist license in good standing.
• Graduate of an accredited physical therapy program.
• Minimum of one year of physical therapy experience.
• Home health experience preferred.
• Current BLS certification.
• Valid Texas driver’s license.
• Reliable transportation and current automobile insurance.
• Strong organizational, communication, and clinical skills.
• Experience with electronic medical records preferred.

Responsibilities

• Evaluate patients and provide skilled physical therapy services according to physician orders and individualized treatment plans.
• Implement therapeutic exercises and interventions to improve mobility, strength, balance, pain control, and functional independence.
• Assess patients’ home environments and recommend equipment and safety modifications to support successful recovery.
• Educate patients and caregivers on home exercise programs, mobility techniques, and fall-prevention strategies.
• Complete timely and accurate clinical documentation in the electronic medical record.
• Collaborate with the interdisciplinary team to coordinate patient care and communicate patient progress.
• Maintain compliance with agency policies, Medicare regulations, and all applicable state and federal standards.
• Travel to patients throughout Fort Worth and the surrounding area.
