Qualifications

• Current state licensure as a Physical Therapist.
• Home health experience preferred but not required.
• Strong communication and interpersonal abilities.
• Ability to work independently and as part of a healthcare team.
• Proficient with electronic documentation systems.

Responsibilities

• Conduct thorough assessments of patients to determine rehabilitation needs.
• Develop and implement individualized treatment plans with the healthcare team.
• Provide hands-on PT interventions to improve strength, flexibility, balance and coordination.
• Educate patients and families on exercises, mobility aids, and independence strategies.
• Monitor and document patient progress, adjusting plans as needed.
• Collaborate with physicians, nurses and OTs for comprehensive care.
• Maintain accurate patient records.
• Communicate with patients, families, and caregivers to foster a supportive environment.

Position Overview:

As a Home Health Physical Therapist, you will play a crucial role in providing specialized rehabilitation services to our patients within the familiar and supportive environment of their homes. You will work closely with patients to assess their physical condition, develop personalized treatment plans, and implement therapeutic exercises to promote recovery, improve mobility, and enhance overall well-being.

This is a PRN position (NOT FULL-TIME), meaning you will be working with us on an as-needed basis. You will be hired as a W2 employee, not as an independent contractor.

Key Responsibilities:

1. Conduct thorough assessments of patients' physical conditions to determine rehabilitation needs.
2. Develop and implement individualized treatment plans in collaboration with the healthcare team.
3. Provide hands-on physical therapy interventions to improve patients' strength, flexibility, balance, and coordination.
4. Educate patients and their families on therapeutic exercises, mobility aids, and strategies to promote independent living.
5. Monitor and document patients' progress, adjusting treatment plans as necessary.
6. Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care.
7. Maintain accurate and up-to-date patient records

Qualifications:

1. Current state licensure as a Physical Therapist.
2. Prior experience in home health is preferred, but not required
3. Strong clinical and critical thinking skills.

5. Ability to work independently and as part of a collaborative healthcare team.
6. Proficient in Microsoft Office Suite and comfortable using technology and electronic documentation systems.
