Job summary

Riverside Regional Home Health Agency Smithfield seeks a licensed Physical Therapist to perform comprehensive evaluations, diagnose PT conditions, and develop effective plans of care to restore function and mobility. You will deliver therapy services and ensure outcomes align with evidence-based guidelines.

You will supervise PT Assistants and Home Health Aides, conduct OASIS assessments, and participate in interdisciplinary team meetings to optimize patient care and discharge planning.

Qualifications

• Licensed physical therapist capable of performing evaluations and developing plans of care.
• Must be able to conduct OASIS assessments and timely re-evaluations.
• Experience supervising PT assistants and home health aides is preferred.

Responsibilities

• Performs evaluations, determines PT diagnoses, develops plan of care, and performs PT procedures.

Smithfield, VirginiaHiring Range$55.00 - $63.00/Hourly Actual pay is determined by program, work commitment and business needsOverviewThe Physical Therapist will follow appropriate practices, to perform evaluations, timely re-evaluations and OASIS assessments, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual. The Therapist is required to be flexible with daily scheduling and may be required to work various patient care settings.What you will doPerforms evaluations, determines PT diagnoses, develops plan of care, and performs physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. Evaluates and assesses the patient timely, based on principles outlined by practice guidelines, evidence based guidelines and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc.Provides reassessments including OASIS assessments at appropriate time points to determine the level of functioning. Updates and revises the plan of care in consultation with the physician and interdisciplinary team members.Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings, as assigned.Supervises Physical Therapy Assistants and Home Health Aides in providing services including completion of required supervisory visit documentation. Participates in training and support of all team members, volunteers and students.Provides therapeutic treatments and evaluation of equipment needs as applicable. Prepares clinical and progress notes that support skilled intervention and assesses outcomes of care.
