Job summary

Rendevor Dialysis seeks an experienced Registered Nurse to oversee the dialysis unit operations within a post-acute facility. You will ensure quality patient care, regulatory compliance, and staff education across the unit.

The role requires an active Texas RN license, ADN/BSN (MSN preferred), and dialysis experience. Strong English communication, computer skills, and on-call availability are essential for success.

Qualifications

• ADN or BSN required; MSN preferred.
• Current state RN license in Texas.
• CPR certification (AHA BLS).
• Minimum 18 months dialysis nursing experience in last 24 months.

Responsibilities

• Oversee dialysis unit operations ensuring regulatory compliance (ESRD/HCFA, OSHA).
• Provide clinical oversight and education to staff and non-licensed personnel.
• Coordinate care across settings and develop care plans with the patient care team.
• Participate in quality improvement and staff training; ensure safety and infection control.

What Youll Do

Oversee the complete operation and performance of the delivery of dialysis treatment and care under the terms of the contract between Rendevor Dialysis and the Skilled Nursing Facility within state and federal regulations Use professional nursing knowledge critical thinking skills and dialysis experience to serve as a resource to staff Physicians and Physicians Assistants

Minimum Qualifications

Education and Experience Graduate of an accredited nursing program required; Associates Degree in Nursing ADN Bachelor of Science in Nurse BSN andor Master of Nursing MSNCurrent state licensure as a Registered NurseCurrent CPR certification AHA BLS18 months of clinical nursing experience to patients on maintenance dialysis within the last 24 months

Required Skills and Competencies

To maintain on call status on an intermittent or regularly scheduled basisAbility to satisfactorily communicate in English including reading writing speaking and understandingComputer proficiency

Essential Duties and Responsibilities

In conjunction with the Vice President ensures dialysis unit operates in compliance with all ESRD HCFA federal AAMI OSHA and company policies and proceduresResponsible for training and development of Skilled Nursing Facility staff while providing clinical oversight to non licensed support team delegating tasks as appropriateKnows and follows policies and procedures under CMS ESRD and state regulations and Rendevor Dialysis monitors staff compliance of sameMonitors and assures patients receive adequate medical attention during dialysis treatments and pertaining to dialysis care while a patient of the Skilled Nursing Facility in the dialysis unitConducts comprehensive assessments that include the medical behavioral pharmaceutical and social needs of the patient identify gaps in care and barriers to good healthBased on this assessment and in conjunction with the patient care team create and implement a care plan that will address identified needs remove barriers to care and improve the health of the patientCoordinates care by serving as the advocate and resource for the patient their family and their providersFacilitates continuity of care spanning settings such as the home hospital skilled nursing facility and acute care facilityManages patients during periods of transitions of care minimize avoidable readmissionsAssesses the patients knowledge of their renal condition and provide education and self management supportProvides ongoing reassessment and follow up to improve patient outcomesReviews and maintains updated policies and procedures on a regular basisDevelops and monitors quality control measures to assure continuous quality of patient careFacilitates Quality Assurance MeetingsParticipates in the IDT and QAPI committee and attend meetings as needed andor requestedIdentifies and reports concerns issues and opportunities for improvement when identifiedParticipates in performance improvement projects within licensure certification and scope of practiceParticipates in annual QAPI training and other continuing education as assigned and requiredProvides suggestions for alternate strategies for indicators not improvingAssists in building and maintaining a facilityclinic culture of safety and qualityReports errors and near misses so that steps can be taken to prevent recurrenceReports any negative feedback complaints grievances from other staff members patients and familiesUses evidence based guidelines and proper infection control practicesMaintains open communication with Skilled Nursing Facility Administrator Medical Director and Rendevor Dialysis management and staffUtilizes resources to improve skills knowledge and practice to assure continuous quality of patient carePromotes a teamwork approach by offering information advice and assistance to all unit staff members in a positive courteous and cooperative mannerChecks email at least weekly and responds appropriately and timely ; uses company provided email for all work related email communicationsMaintains regular and predictable attendance and punctualityAdheres to all company policies and proceduresPerforms other duties as assigned

Work Environment

This job exists in the medical unit of a post acute facility This role may include exposure to chemicals and blood borne pathogens This role routinely involves the use of both medical and standard office equipment

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job While performing the duties of this job the employee is regularly required to talk or hear The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms This position requires the ability to push pull or lift supplies and products up to 50 pounds Specific visual abilities required by this job include close vision and ability to adjust focus The employee is required to work the full schedule which may range from five to fifteen hours in any given day based on the unit requirements ie patient census staffing needs etc
