Job summary

Concerto Renal Services is seeking a PRN Licensed Practical Nurse to join its team in providing safe and effective dialysis therapy under the supervision of a Registered Nurse. You will perform direct patient care, monitor vitals, operate dialysis machines, document treatments, and maintain a clean, safe environment while delivering compassionate service to patients and families.

The role involves teamwork with the dialysis health care team and adherence to state and federal regulations.

Qualifications

• Licensed Practical Nurse required.
• Experience in dialysis settings preferred.

Responsibilities

• Direct Patient Care: Provides hands-on care to assigned patients under the supervision of a licensed nurse, including initiating and discontinuing dialysis treatments per prescribed orders.
• Vital Signs & Monitoring: Obtain pre and post treatment vitals, weight and vascular access assessment. Report any unusual changes to the RN.
• Dialysis Machine Operation: Sets up, operates, and performs safety checks on hemodialysis machines; administers heparin as permitted by state law.
• Documentation & Lab Work: Accurately documents patient treatment details including vital signs, treatment parameters, and changes in condition.
• Customer Service & Environment: Delivers excellent service to patients, families, and staff, while maintaining a clean and safe work environment.

Concerto is seeking a PRN Licensed Practical Nurse to join its team! The PRN Licensed Practical Nurse functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients. Works under the supervision of a Registered nurse in accordance with Concerto’s policies, procedures, and training and in compliance with regulations set for by state and federal agencies.

What Will You Do?

1. Direct Patient Care

   Provides hands-on care to assigned patients under the supervision of a licensed nurse, including initiating and discontinuing dialysis treatments per prescribed orders.

2. Vital Signs & Monitoring

   Obtain pre and post treatment vitals, weight and vascular access assessment. Report any unusual changes or findings to the Registered nurse.

3. Dialysis Machine Operation

   Sets up, operates, and performs safety checks on hemodialysis machines; administers heparin as permitted by state law. Initiate and discontinue dialysis treatment according to prescribed orders and policy.

4. Documentation & Lab Work

   Accurately documents patient treatment details including vital signs, treatment parameters, and change in condition, and prepares laboratory specimens as prescribed.

5. Customer Service & Environment

   Delivers excellent service to patients, families, and staff, while maintaining a clean and safe work environment
