Job summary

Continuum Therapy Partners is seeking licensed Physical Therapists to join our therapist-led network. This PRN daytime role offers the chance to impact patient outcomes across our Middletown facilities.

You will evaluate and implement individualized treatment plans, document progress, and educate patients and families on home exercise programs. A DPT and active state license are required, with NPTE completed.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; NPTE must be passed.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
