Job summary

Outcome HC is seeking a PRN Certified Occupational Therapy Assistant (COTA) to join our long-term care/ skilled nursing team in New Jersey. You will work with the supervising OT to implement individualized plans, guide residents through therapeutic exercises, and document progress.

The role emphasizes compassionate, reliable care, strong teamwork, and adherence to facility policies and regulatory requirements to support resident independence and mobility.

Qualifications

• Current COTA license/certification in New Jersey (NJ).
• Graduate of an accredited Occupational Therapy Assistant program.
• Previous long-term care or skilled nursing experience preferred.
• Strong communication and teamwork skills.
• Compassionate, reliable, and professional demeanor.

Responsibilities

• Assist the Occupational Therapist in implementing individualized treatment plans.
• Provide therapeutic exercises and activities to residents.
• Monitor and document resident progress and report changes to the therapy team.
• Maintain accurate and timely documentation.
• Encourage residents and provide compassionate patient-centered care.
• Ensure compliance with facility policies, state, and federal regulations.
