Job summary

State of Arizona is seeking a CNA to provide para-professional nursing care to Veterans/Members in a facility setting. The role includes assisting with ADLs, transfers, dining, and basic medical tasks under supervision, while documenting care and supporting AMS/QAPI processes.

You will work with nursing staff to monitor health status, operate essential equipment, keep a clean environment, and communicate effectively to ensure timely care and patient comfort.

Qualifications

• Knowledge of para-professional nursing care practices and safety procedures.
• Familiarity with state and agency nursing regulations and compliance.
• Experience in using medical equipment for vital signs and basic care.
• Ability to maintain a clean, safe, and hygienic patient environment.

Responsibilities

• Assist Veterans/Members with transfers, ambulation, eating, bathing, dressing, toileting and personal hygiene.
• Support in basic procedures and devices to strengthen limbs and flexibility.
• Assist with post-surgical and post-mmortem care under supervision.
• Monitor physical and mental status using medical equipment and document vitals and samples.
• Record and report documentation and simple reports in medical records.
• Assist dining and transport to meals, communicate choices to dining staff.
• Report to licensed nurses in emergencies and follow procedures.
• Perform clerical duties and help with appointments and AMS processes.
• Engage in QAPI activities and standard work as part of quality improvement.

Job description

• -Assists Veterans/Members with transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene and other ADLs.
• -Follows instructions to use simple procedures and devices to strengthen Veterans'/Members' limbs and maintain flexibility.
• -Assists nursing staff with simple medical procedures such as suppositories, enemas, treatments, post-surgical care and post-mortem care.
• -Assists in monitoring physical and mental status of Veterans/Members by utilizing appropriate medical equipment and techniques to take vital signs (temperature, pulse, respirations); obtain urine, sputum or stool samples; and track input/output.
• -Responsible for medical record and worksheet documentation on all assigned Veterans/Members. Keeps records and prepares simple reports.
• -Assists Veterans/Members with dining to include transporting to the dining room, informing them about menu choices available for the day, communicating dining staff about Veterans'/Members' choices and serves Veterans/Members their meals in courses, assisting them, if necessary, Provides resident care and comfort by responding to patient requests (verbal or call light) for assistance timely in accordance with the Resident Bill of Rights.
• -Maintains cleanliness of areas neat and sanitary by changing bedding, linens and other items.
• -Reports to and assists licensed nurses in emergency situations rapidly and appropriately following established policies and procedures.
• -Performs other related duties such as transporting Veterans/Members to appointments outside the facility and clerical duties (e.g., answering phones, filing, etc.) as needed.
• -Actively involved in AMS processes such Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work.
• -Participates fully in the QAPI process, our Quality Assurance and Performance Improvement program mandate by CMS which incorporates AMS.
