Job summary

I3 INFOTEK INC seeks a Certified Nursing Assistant to support Veterans/Members with ADLs and daily care in a long-term care setting. You will assist with transfers, mobility, meals, and personal care while following CMS and AMS guidelines.

The role requires AZ CNA certification, a 2-step TB test, and LTC experience. Day or night shifts on a 12-hour schedule are available; 40 hours/week may not be guaranteed.

Qualifications

• Knowledge of para-professional nursing care and services.
• Familiar with medical equipment use and safety procedures.
• Understanding of state regulations governing para-professional care.

Responsibilities

• Assist with ADLs: transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene.
• Monitor vital signs and collect urine, stool samples as needed.
• Document medical records and worksheets accurately and timely.
• Assist in dining: transport to meals, inform choices, serve courses.
• Provide timely resident care and comfort per rights and requests.
• Maintain a clean, neat, sanitary environment; report emergencies promptly.

Job description

• Assists Veterans/Members with transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene and other ADLs.
• Follows instructions to use simple procedures and devices to strengthen Veterans'/Members' limbs and maintain flexibility.
• Assists nursing staff with simple medical procedures such as suppositories, enemas, treatments, post-surgical care and post-mortem care.
• Assists in monitoring physical and mental status of Veterans/Members by utilizing appropriate medical equipment and techniques to take vital signs (temperature, pulse, respirations); obtain urine, sputum or stool samples; and track input/output.
• Responsible for medical record and worksheet documentation on all assigned Veterans/Members. Keeps records and prepares simple reports.
• Assists Veterans/Members with dining, including:
• Transporting them to the dining room
• Informing them about menu choices available for the day
• Communicating dining staff about Veterans'/Members' choices
• Serving Veterans/Members their meals in courses
• Assisting them when necessary
• Provides resident care and comfort by responding to patient requests, verbal or call light, for assistance timely and in accordance with the Resident Bill of Rights.
• Maintains cleanliness of areas neat and sanitary by changing bedding, linens and other items.
• Reports to and assists licensed nurses in emergency situations rapidly and appropriately, following established policies and procedures.
• Performs other related duties such as:
• Transporting Veterans/Members to appointments outside the facility
• Clerical duties, such as answering phones and filing, as needed.
• Actively involved in Arizona Management System (AMS) processes such as:
• Huddle Boards
• Problem Solving

• Success Tracking
• Participates fully in the QAPI (Quality Assurance and Performance Improvement) process, the quality assurance and performance improvement program mandated by CMS, which incorporates AMS.

Knowledge of:

• State and agency laws and regulations governing para-professional nursing practices.
• Practices and procedures of using medical equipment in assigned duties.

• Active Certified Nursing Assistant Certification in the State of Arizona
