Job summary

Frontpoint Health, LLC is seeking a PRN Certified Nursing Assistant (CNA) to provide compassionate in-home care. The ideal candidate will perform personal care, monitor vital signs, and support patients and families in their homes.

This role requires Texas CNA certification, reliability, strong communication, a valid driver’s license, and the ability to work independently or with a team. PRN shifts may include evenings and weekends.

Qualifications

• Current certification as a Certified Nursing Assistant (CNA) in the state of Texas.
• Experience working as a Home Health Aide or in a similar role.
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation.
• Ability to pass a background check and drug screening.

Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, and grooming.
• Provide medication reminders and assist with medication administration as directed by the patient's healthcare provider.
• Assist with mobility and transfers, including the use of assistive devices such as walkers and wheelchairs.
• Monitor and record vital signs, such as blood pressure and temperature
• Assist with light housekeeping tasks, such as laundry and meal preparation.
• Provide companionship and emotional support to patients and their families.
• Report any changes in the patient's condition to the healthcare provider.
