Job summary

Mental Health Association of Rochester seeks a Care Manager to join an interdisciplinary team. You will assess patient needs, plan and coordinate transitions from admission to discharge, and advocate for patients during hospitalization and after discharge.

The ideal candidate is a licensed nurse with 2–3 years in acute care and BLS/CPR certification; care management certification is preferred.

Qualifications

• Graduate of an accredited School of Nursing; Bachelor's Degree in Nursing required.
• Current state licensure required.
• BLS/CPR certification required or obtained within 90 days.
• 2-3 years of experience in an acute care hospital setting preferred.
• Care Management or Utilization Management experience and certification preferred.
