Qualifications

• DDS or DMD from an accredited dental school.
• Indiana State License to Practice Dentistry required.
• CSR and DEA certificate required.

Responsibilities

• Obtain complete dental history and physical data on patients.
• Perform a complete dental exam and record findings.
• Manage acute and long-term dental needs of patients.
• Adhere to HIPAA rules and patient confidentiality.
• Maintain patient records, charts and related information.
• Order laboratory and diagnostic procedures and evaluate results.
• Develop preliminary diagnoses and therapeutic plans.
• Administer medications and injections per licensing.
• Educate patients on oral health and treatment instructions.
