Formación

• Must hold a valid New Jersey CHHA license.
• High school diploma or equivalent.
• Pass background check and health screening.
• Reliable, compassionate and professional with strong communication.
• Driver’s license and reliable transportation preferred.

Responsabilidades

• Assist clients with daily living activities per care plan.
• Provide companionship and emotional support.
• Perform light housekeeping, meal prep, and medication reminders.
• Follow the nurse’s care plan and report changes.
• Maintain accurate documentation and timely reporting.
• Coordinate with agency staff to ensure quality care.

Responsibilities:

• Assist clients with daily living activities: bathing, dressing, grooming, toileting, and mobility

• Provide companionship and emotional support

• Perform light housekeeping, meal preparation, and medication reminders

• Follow the individualized care plan provided by the nurse

• Report any changes in the client’s condition to the agency

• Maintain accurate and timely documentation

Requirements:
