Responsibilities

• Utilizes nursing process to plan, implement, and evaluate patient care in home setting.
• Initiates care plan and updates per patient status.
• Provides therapeutic, preventive, and rehabilitative nursing actions.
• Intervenes in response to changes in patient condition and communicates with physicians.
• Administers medications and treatments as prescribed and documents effectiveness.
• Reviews care plan and coordinates education, goals, and discharge planning.
