Job summary

Always Compassionate Health is seeking compassionate LPNs and RNs to provide one-on-one nursing care to patients in their homes. We offer competitive pay, flexible scheduling, and immediate openings to make a meaningful difference in patients’ lives.

Responsibilities include administering medications, performing G-tube/PEG and tracheostomy care, monitoring patient conditions, and educating patients and caregivers while maintaining proper documentation.

Qualifications

• Active New York State LPN or RN license.
• Minimum of 6 months of home care experience.
• Current CPR/BLS certification.
• Ability to meet physical requirements, including lifting up to 50 pounds.

Responsibilities

• Provide skilled nursing care and administer prescribed medications and treatments.
• Perform G-tube/PEG care, tracheostomy care, and suctioning as required.
• Monitor patient conditions and report changes promptly.
• Provide ventilator support when required and appropriately trained.
• Educate patients and caregivers and maintain accurate documentation.
