Job summary

First Day Homecare is seeking a part-time registered nurse or licensed practical nurse to support a school-aged child with a tracheostomy in Fort Wayne, Indiana. You will provide skilled nursing care under medical orders and coordinate with the clinical supervisor and care team.

Responsibilities include administering medications, following the care plan, documenting changes, and ensuring a safe environment. A pediatric private-duty setting offers meaningful work and flexible scheduling.

Qualifications

• Must graduate from an accredited school of nursing.
• Current RN or LPN license in Indiana.
• Current CPR certification.
• Strong written and verbal communication skills.
• Licensed driver with valid auto insurance.

Responsibilities

• Provide professional nursing care per Nurse Practice Act and orders.
• Follow medical practitioner orders and client's care plan.
• Document changes in orders in the medical record per agency procedures.
• Perform pediatric services including tracheostomy care, ventilator management, and tube feeding.
• Collaborate with the care team and maintain a safe environment.
• Adhere to agency policies and participate in care coordination.
• Attend agency meetings as requested and perform related duties.

Job Summary

Performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of professional standards and the Nurse Practice Act. All nursing services provided will be under the direction of medical practitioner (MD, DO, NP, PA, etc.) orders.

Reports To: Clinical Supervisor
Provides Supervision To: NA – No supervision responsibilities

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care in accordance with medical practitioner orders and the client’s care plan.
• Maintains orders for clients and documents all changes in orders in the medical record following Agency procedures.
• The scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Works collaboratively with other members of the client’s care team.
• Promotes personal safety and a safe environment for clients and coworkers.
• Performs job in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
• Participates in the coordination of care and attends Agency meetings as requested.
• Performs other related duties and responsibilities as deemed necessary.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current license to practice as an RN or LPN in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements.
