First Day Homecare is seeking an experienced Registered Nurse or Licensed Practical Nurse to provide skilled nursing services in client homes under medical practitioner orders. You will work under the supervision of a Clinical Supervisor and collaborate with a multidisciplinary care team to deliver high-quality care.

The role requires compliance with state Nurse Practice Act, accurate documentation, and the ability to manage complex pediatric cases, including specialized tasks.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN or LPN license in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.

Responsabilités

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Delivers care per medical practitioner orders and client care plan.
• Maintains client orders and documents changes in the medical record.
• Clinical scope includes personal care, monitoring, medication administration, and client education.
• Collaborates with the care team to ensure quality care.
• Promotes safety for clients and coworkers.
• Adheres to agency policies and accreditation standards.
• Participates in care coordination and agency meetings.

Job Summary

Performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of professional standards and the Nurse Practice Act. All nursing services provided will be under the direction of medical practitioner (MD, DO, NP, PA, etc.) orders.

Reports To: Clinical Supervisor
Provides Supervision To: NA - No supervision responsibilities

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care in accordance with medical practitioner orders and the client's care plan.
• Maintains orders for clients and documents all changes in orders in the medical record following Agency procedures.
• The scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Works collaboratively with other members of the client's care team.
• Promotes personal safety and a safe environment for clients and coworkers.
• Performs job in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
• Participates in the coordination of care and attends Agency meetings as requested.
• Performs other related duties and responsibilities as deemed necessary.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current license to practice as an RN or LPN in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements.
