Qualifications

• MN LPN or RN credential required.
• Experience in private duty/home care preferred.
• Ability to work independently and as part of a care team.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor vital signs and report changes to the Nurse Administrator.
• Assist in developing and implementing individualized care plans.
• Provide personal care such as bathing, dressing, and grooming.
• Support acute pain management strategies.
• Maintain accurate patient documentation.
• Collaborate with interdisciplinary teams to improve patient outcomes.
• Educate patients and families on wellness and health management.
