Aveanna Healthcare in Texas seeks a Licensed Practical Nurse (LPN/LVN) to provide skilled nursing care in patients' homes under RN supervision. You will document care plans and coordinate with families and clinicians to ensure high-quality outcomes.

The role offers flexible full-time, part-time, and PRN schedules, with local support, electronic documentation tools, and opportunities for skill advancement. Active LPN/LVN license and CPR are required.

Qualifications

• Active, unencumbered LPN/LVN license in the state of practice.
• Current CPR certification.
• TB test within last 12 months.
• Six months prior hands-on nursing experience preferred but not required.
• Reliable transportation required.

Responsabilités

• Deliver and coordinate quality patient care per physician orders.
• Observe, assess patient condition and report changes to supervisor/physician.
• Document all nursing activities promptly in patient records.
• Participate and update the nursing care plan.
• Provide care to meet expected outcomes; act on changes.
• Identify changes requiring intervention and implement care.
• Adhere to policies, state/federal regulations, and Nurse Practice Act.
• Maintain infection control per OSHA standards.
• Educate patient and family about disease, self-care and prevention.
• Pursue ongoing education and skill evaluations.
