Job summary

Personal Touch Home Health Care Services Inc. is seeking a Private Duty Nurse (LPN/LVN) to provide one-on-one skilled nursing care in patients’ homes. The role includes patient assessments, medication administration, ventilator and tracheostomy care, and coordination with the clinical team.

You will educate families, document all care and observations, and follow individualized care plans to ensure high-quality home health services.

Qualifications

• Current LPN/LVN licensure.
• Strong clinical judgment and patient-focused care approach.
• Ability to perform medication administration and treatment procedures.
• Experience or capability to manage ventilator and tracheostomy care.
