Job summary

Elk Valley Health Services, part of LHC Group and within the Optum family, is seeking a Licensed Practical Nurse to provide direct in-home nursing care under physician orders in the geographic service area. You will document visits and coordinate patient care.

Requirements include LPN licensure, valid driver's license and vehicle, and ability to work with diverse patients. Preferred CPR, flexibility, multitasking, independence, and strong communication.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice, per plan of care, in coordination with the health care team.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates needs and changes in condition to the team.
• Function in any home situation regardless of patient demographics.
