Job summary

At Home Healthcare of Middle Tennessee is seeking a qualified nurse (LPN or RN) to provide skilled in-home care across Kentucky. You will deliver care per the service plan, follow physicians' orders, and adhere to agency policies while prioritizing patient safety.

Responsibilities include teaching patients, administering medications per age and condition, completing progress notes, coordinating with the Director of Patient Care Services, and participating in the on-call schedule.

Qualifications

• Valid Kentucky nursing license (LPN or RN) is required.
• CPR certification is required or willingness to obtain.
• TB skin test or willingness to obtain is required.

Responsibilities

• Provides skilled nursing care in the home to patients per service plan and agency policies.
• Maintains patient safety and reduces health care errors.
• Promotes patient health through teaching and rehabilitative measures.
• Assists patients in learning self-care techniques.
• Completes progress notes and follows the client service plan.
• Submits notes and visit verifications promptly.
• Keeps abreast of nursing trends and knowledge.
• Communicates patient status to staff.
• Participates in on-call schedule.
• Maintains confidentiality of all records.
