Azza HealthCare Agency is seeking a compassionate Private Duty Nurse (LPN) to deliver one-on-one skilled nursing in patients' homes. You will perform assessments, monitor health, administer medications, provide wound care, and support families.

You will handle trach/vent and G-tube care as needed, collaborate with families and care team to implement individualized plans, and document care accurately while responding to changes in patient condition.
