Qualifications

• Bachelor's degree in Psychology, Social Work, or Mental Health/Behavioral Science required.
• Criminal Justice degree accepted with psychology coursework and two years MH case management experience.
• Experience with case management, counseling and liaison roles preferred.

Responsibilities

• Conduct social history assessments and monitor mental health status.
• Provide case management, social and habilitative services, and counseling.
• Serve as liaison with program services for patients with mental health needs.
• Coordinate treatment and aftercare planning with departments and community agencies.
• Administer and score psychometric tests under supervision.
• Perform related duties and participate in QI/QM programs.
