Qualifications

• CT RN license required.
• Minimum of 1 year RN experience; homecare experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Facilitate communication between the patient and the interdisciplinary team.
