Qualifications

• Massachusetts RN license and BLS CPR certification required.
• Minimum of 2 years nursing experience.
• Experience delivering clinical care, education, and treatment to patients with substance use or primary care needs.
• Demonstrated ability to work with patients from diverse backgrounds.
• Excellent verbal and written communication and interpersonal skills.

Responsibilities

• Provides independent quality triage and patient care with strong assessment skills.
• Provides education, ongoing assessment and support to patients with chronic substance use health conditions.
• Develops and implements care plans for complex patients in collaboration with patients and medical staff.
• Triages and coordinates patient care to ensure access and patient success.
• Promotes health and wellness for individuals and groups of patients.
• Advocates for patients’ rights, values and beliefs.
