Job summary

Oak Street Health is seeking a Registered Nurse to deliver team-based primary care to adults on Medicare in underserved communities. You’ll perform assessments, triage, chronic disease management, vaccinations, and patient education while collaborating with physicians, social workers, and care managers.

You’ll use EHRs to coordinate care, support population health initiatives, and improve outcomes in a patient-centered, inclusive culture that fosters professional growth and lasting community

Qualifications

• Active RN license in the state of practice.
• Experience in primary care or care coordination is a plus.
• Comfort with Medicare populations and community health initiatives.
