Town Square Health is seeking a Primary Care Physician (MD/DO) for our Rochester, NY location. You will deliver high-quality primary care in a Medicare-focused, value-based environment with dedicated Navigators, longer visit times, and a supportive team-based model.

Join early and help build the care model from the ground up, with real ownership and influence over clinical workflows and patient outcomes. In-person, clinic-based role with full-time hours.

Qualifications

• MD or DO degree required.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics.
• Active New York State medical license in good standing or must obtain prior to start date.
• DEA registration prior to hire/start date.
• Must meet all employer and payer credentialing requirements.

Responsabilités

• Deliver high-quality primary care with time and team support in a clinic-based setting.
• Lead a team-based care model and coordinate with Navigators and specialty colleagues.
• Build and shape clinical workflows for a new clinic and contribute to care model development.
• Partner with Navigators to support patients between visits using AI-enabled workflows.
• Ensure compliance with post-offer requirements and credentialing processes.
