Job summary

AbsoluteCARE Medical Center & Pharmacy in Chicago seeks a licensed physician to join our outpatient team. You will provide counseling on settings, perform diagnostic evaluations, and guide patients through specialist referrals and end-of-life programs as part of a large integrated care team.

The role emphasizes chronic disease management, coordination with social work, care management, behavioral health, nursing, pharmacy, and nutrition, and use of EMR to improve quality metrics.

Qualifications

• Licensed physician in IL with current license.
• Board-certified in Internal Medicine or Family Practice.
• BLS certification required.
• Experience in population health management preferred.
• Comfort working with psychosocially and medically complex patients.

Responsibilities

• Focus on management and improvement of health for patients with multiple chronic conditions.
• Work in a team environment with care management, behavioral health, nursing, pharmacy, and others to discuss and implement plans of care.
• Treat acute exacerbations onsite as needed.
• Differentiate when a higher level of care is required.
• Provide same day/next day or urgent appointments for discharged or home patients.
• Track patients and disease processes to prevent ER visits and readmissions.
• Educate patients about chronic diseases and healthy choices.
• Practice evidence-based medicine per PCMH model.
• Meet quality metric goals and participate in team meetings.
• Provide on-call coverage for office and hospital in rotation.
