AllCare is expanding to New Jersey and seeks an MD/DO physician who owns a Medicare-certified primary care clinic in Bergen or Morris County. The role offers part-time, on-the-road visits with patients in senior living communities and uses an AI-powered care coordination platform for CCM, APCM, and PCM.

Qualifications

• MD/DO owner of a Medicare-certified primary care clinic (Bergen or Morris County).
• Active New Jersey medical license and DEA registration.
• Board-certified or eligible in Internal Medicine, Family Medicine, or Geriatrics.

Responsibilities

• Lead physician leadership with CCM program participation and patient visits.
• Coordinate care management programs and scheduling via AllCare.ai platform.
• Provide in-clinic or on-site visits at partner senior living communities.

AllCare.ai is building the care model for that wave: mobile primary care and monthly care management programs (CCM, APCM and PCM) for seniors where they live, run on our own AI-powered care coordination platform. In under two years we've grown to 2,500 seniors in 150 senior living communities across California.

Now we're expanding to New Jersey, and we're selecting a limited number of partner clinics in Bergen and Morris counties. What if your practice could add up to $250,000 a year for about 4–5 hours a week of your time?*

How it works

• We partner with local assisted living residences and personal care homes, and our experienced NPs care for their residents through your practice.
• Our team and AI platform run your Medicare care management programs (CCM, APCM and PCM), plus scheduling, documentation and billing.
• You stay the owner. Everything bills through your clinic, with no new staff and no new overhead.

You're a fit if you

• Are an MD/DO who owns a Medicare-certified primary care clinic (1–2 locations) in Bergen or Morris County
• Have Medicare set up as payee and hold an active New Jersey license and DEA registration
• Are board certified or eligible in Internal Medicine, Family Medicine or Geriatrics
