Job summary

Crossover Health is seeking a Physician to lead a care team and deliver high-quality primary health to a patient panel. The physician provides medical oversight, expertise, and leadership to ensure affordable care delivery.

Responsibilities include patient-centered primary care, on-call coverage, and engagement in quality improvement and compliance programs, with a focus on collaboration and adoption of new health technologies within an employer-based health model.

Qualifications

• Residency in Family Medicine or Internal Medicine (ACGME approved).
• Current medical license active in the state of work.
• Current BLS certification.
• Current DEA Registration.
• Three professional references.

Responsibilities

• Provides patient-centered, comprehensive primary care in line with patient-centered medical home model.
• Offers medical and technical Q&A to patients, care staff, and ancillary care providers.
• Provides after-hours coverage in rotation with other market clinicians.
• Participates in practice quality improvement programs and evaluates performance metrics.
• Adheres to privacy, compliance, quality programs, and Code of Conduct.
• Supports implementation of new health technologies, products, and services.
• Performs other duties as assigned.
