HMFP at Beth Israel Deaconess Medical Center seeks a physician to provide medical care in skilled nursing facilities, rest homes, assisted livings, and home settings as part of the Extended Care Community Program (ECCP).

Program Summary Beth Israel Lahey Health Primary Care’s Extended Care Community Program (ECCP) provides patients with personalized, high quality medical care within rehabilitation and long-term facilities. We provide patients with the essential medical care during their stay and support them in transitioning back to their primary care physician upon discharge home or to another appropriate care setting. ECCP physicians and nurse practitioners are board-certified seasoned caregivers who are committed to working with patients, families, as well as the facilities' team members, to provide comprehensive and coordinated patient care. Reports to the Extended Care Community Program’s medical director and (director of operations?). The primary care physician role for the Extended Care Community Program is responsible for providing medical care services at skilled nursing facilities, rest homes, assisted livings and home settings. The physician will be responsible with working collaboratively with ECCP nurse practitioner(s) and interdisciplinary teams at the skilled nursing facilities. Graduate degree from an accredited School of Medicine and Residency Program Qualified candidates should be board-certified/board-eligible in internal medicine or family medicine with a focus on geriatric medicine. Must possess a valid medical license Demonstrated ability to use medical record systems to thoroughly and properly document patient visits.

Pay Range: $250,000 - $290,000 The base pay range reflects what Harvard Medical Faculty Physicians at Beth Israel Deaconess Medical Center (HMFP) reasonably and in good faith expects to pay for this role at the time of posting and may be modified from time to time. Actual compensation within this range may be determined based on several factors, including academic appointment, work experience, specialty training, geography of work location, anticipated productivity, FTE basis, and role expectations. In addition to base compensation, this role may be eligible for performance-based incentives, which may include bonuses for productivity and quality.
