Qualifications

• Doctor of Medicine Degree or its foreign equivalent.
• Current and valid Physician License from the State of Indiana at time of hire.
• DEA licensure and prescriptive authority from the State of Indiana at time of hire.
• Board Certified or Board Eligible in a related field.
