Responsibilities

• Deliver comprehensive adult and/or pediatric care, including preventive services, chronic disease management, and urgent care.
• Supervise and coordinate with support staff to ensure effective, team-based patient care.
• Collaborate with behavioral health and dental providers to support integrated care.
• Document clinical encounters and care plans in the EHR.
• Participate in quality improvement activities to enhance care outcomes.
