Job summary

You will care for Veterans, conduct exams, take histories, order tests, and coordinate care as part of the Patient Aligned Care Team (PACT). You will work with other clinicians to deliver patient-centered care, attend team meetings, document timely notes, and support a system-wide approach to Whole Health.

Responsibilities

• Provide primary care to Veterans in outpatient setting
• Perform physical examinations and obtain medical histories
• Order and interpret tests and manage treatment plans
• Coordinate care within the Patient Aligned Care Team (PACT)
• Participate in team huddles and system redesign projects
