Job summary

Hartford HealthCare Primary Care in Shelton, CT, and Monroe, CT, is hiring a Nurse Practitioner (APRN) or Physician Assistant (PA-C) for 30 hours per week in a 100% outpatient adult primary care setting.

Role focuses on preventive care, wellness, and chronic disease management within a collaborative team of physicians and nurses. Part-time, benefit-eligible position with no on-call requirement.

Qualifications

• CT medical license (or eligible) is required.
• APRN or PA-C training is required.

Responsibilities

• Under established guidelines and in collaboration with physicians, Nurse Practitioners and Physician Assistants perform routine physical examinations and develop medical, educational, and psychosocial plans.
• Coordinate with care team to determine need for referrals to specialists.
• Provide treatment within the clinician's scope of practice and refer outside as needed.

Nurse Practitioners And Physician Assistants Role Scope

• Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
• In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
• Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the clinician’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Qualifications

Requirements:

• Licensure: Unrestricted State of Connecticut Medical License (or eligible)
• Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA/PA-C
• Experience: 2+ years’ experience in a primary care or relevant outpatient setting
