Job summary

Northshore Home Health Care in San Francisco seeks a board-certified internist or family physician to provide comprehensive primary care in a collaborative, tech-enabled clinic environment, serving adults through in-person and virtual visits.

You will manage preventive care, acute visits, and chronic disease, while coordinating care with NPs, PAs, RNs, and behavioral health teams to ensure timely access and high-quality outcomes.

Qualifications

• Board-certified or eligible in Internal Medicine or Family Medicine.
• Comfortable using technology and virtual care tools.
• Strong clinical judgment, patient communication, and preventive focus.
• Motivated to improve primary care using data, innovation, and human-centered design.

Responsibilities

• Deliver comprehensive preventive care, acute visits, and ongoing chronic disease management.
• Maintain and care for an assigned adult patient panel.
• Ensure timely access with same- or next-day support.
• Coordinate care across specialties and community resources.
• Conduct comprehensive exams and clinical evaluations.

• Participate in quality improvement and population health initiatives.
