Job summary

Outreach Community Health Centers seeks a dedicated primary care physician to deliver quality medical care for adults and children in a community health setting in Racine, Wisconsin. You will evaluate and manage conditions, order tests, and coordinate care with specialists.

Job description

Position Summary

The primary care physician provides medical services to adult and pediatric patients in accordance with agency policies and procedures.

Essential Duties and Responsibilities
  • Evaluate, diagnose and manage acute and chronic medical conditions.
  • Order, perform and interpret laboratory and diagnostic tests.
  • Perform diagnostic and therapeutic procedures within provider scope.
  • Provide preventative care and lifestyle counseling.
  • Prescribe medication as necessary and appropriate in accordance with state and federal laws.
  • Provide care with consideration of the patient’s social, financial and cultural context.
  • Actively promote the cooperation and coordination of Primary medical and Specialty Medical care offered to our patients.
  • Respond to medical emergencies and coordinate care transfer to higher acuity facility.
  • Provide after-hours clinical advice for OCHC patients when assigned.
  • Assume quality of care for all patients.
  • Adhere to the highest standards of medical practice, ethics, and professionalism at all times.
  • Assist in the establishment, evaluation and modification of medical policies and procedures, as well as quality improvement efforts and programming.
  • Assist in designing, implementing and evaluating educational programs for patients and clinical staff.
  • Direct and supervise clinical staff, as appropriate.
  • Become familiar with the Federal and State rules, regulations, and practices in a Community Health Center.
  • Complete special projects as assigned.
  • Maintain professional and technical competency and remain current relative to changes in the industry.
  • Ensure the timely completion of all required paperwork, records, documents, etc.
  • Ensure compliance with all safety and work rules and regulations. Ensure the maintenance of departmental housekeeping standards.
Minimum Qualifications (Knowledge, Skills, and Abilities)
  • Handle all situations with tact, persuasiveness and diplomacy.
  • Work in a self-directed manner, managing multiple projects and assignments simultaneously.
  • Possess a high level of confidentiality.
  • Serve as a provider representative for internal and external groups as assigned.
  • Attend and actively participate in scheduled internal meetings.
  • Participate in clinical peer reviews.
  • Carry out supervisory responsibilities in accordance with the organization's policies and applicable laws and regulations.
  • Interview and train employees and contracted staff; plan, assign and direct work; provide clinical supervision; address complaints and participate in problem resolution.
CERTIFICATES, LICENSES, REGISTRATIONS:

Requires Medical Degree and Board Certification or Board eligibility in Family Medicine or Med-Peds, and a valid license to practice medicine in the state of Wisconsin.

Considerations & Statement

Outreach Community Health Centers requires employees in certain departments to be fully vaccinated against MMR (Measles, Mumps, Rubella), Varicella (Chickenpox), and Influenza.
