Job summary

One Brooklyn Health (OBH) is seeking a dedicated Primary Care Physician (Internal Medicine or Family Medicine) to join our growing network in eastern Brooklyn. You will deliver longitudinal care across ambulatory sites for a diverse, underserved population within a fully integrated Epic environment.

The role includes patients with diabetes, hypertension, heart disease and COPD, with telehealth and population health tools to support care coordination and quality improvement.

Qualifications

  • MD or DO degree from an accredited medical school.
  • Completed residency in Internal Medicine or Family Medicine; board-eligible or board-certified.

Responsibilities

  • Provide comprehensive primary care including preventive, chronic, and acute care.
  • Manage complex, multi-morbid patients with urban health needs.

Overview

One Brooklyn Health (OBH) – One of the largest voluntary health systems in the New York metropolitan area is seeking a dedicated and compassionate Primary Care Physician (Internal Medicine or Family Medicine) to join our growing practice network. This is a meaningful opportunity for a clinician committed to delivering high-quality, patient-centered primary care to one of New York City’s most diverse and medically underserved communities.

The Primary Care Physician (“PCP”) will provide comprehensive, longitudinal primary care across OBH’s ambulatory care sites in eastern Brooklyn, serving a high-need patient population with a broad spectrum of acute, chronic, and preventive care needs. OBH operates on Epic, providing a fully integrated electronic health record environment that supports clinical documentation, care coordination, and population health management across all three campuses. We are also connected to many community providers through EPIC Care Connect and we deploy telehealth and remote patient monitoring capabilities.

Responsibilities
Clinical Care:
  • Provide comprehensive primary care services including preventive care, chronic disease management, acute illness evaluation, and care coordination for a panel of adult patients (Internal Medicine) or patients across the lifespan (Family Medicine)
  • Manage complex, multi-morbid patients with conditions including diabetes, hypertension, heart disease, COPD, and behavioral health comorbidities common in an urban safety-net population
  • Conduct in-person and telehealth visits in accordance with OBH ambulatory care protocols and productivity standards
  • Order, interpret, and follow up on diagnostic tests, imaging, and specialist referrals in a timely manner
  • Utilize Epic EHR for clinical documentation, e‑prescribing, care gap closure, and population health reporting
  • Ensure compliance with regulatory, quality, and patient safety standards across all clinical encounters
Population Health & Care Coordination:
  • Actively participate in population health initiatives, including care gap closure, chronic disease registries, and quality measure performance (HEDIS, UDS, and value-based care metrics)
  • Collaborate with care management, social work, behavioral health, and specialty teams to support integrated, whole-person care
  • Support transitions of care for patients discharged from OBH inpatient settings, ensuring continuity and reducing preventable readmissions
  • Engage patients and families in shared decision‑making, health education, and self‑management support
Teaching & Professional Development:
  • Participate in the clinical education of medical students, residents, and advanced practice trainees in collaboration with SUNY Downstate Health Sciences University as well as other medical schools and training programs.
  • Attend departmental meetings, quality improvement sessions, and peer review activities as required
  • Maintain current knowledge of evidence-based primary care guidelines and contribute to a culture of continuous learning and quality improvement
Qualifications
Education:
  • Graduate of a medical school accredited by the Council on Medical Education, required.
  • Completion of an accredited residency program in Internal Medicine or Family Medicine, required.
Experience:
  • None
Licenses, Certifications:
  • Licensed (or eligible to obtain prior to start) by the New York State Department of Education, required.
  • Board certification or board eligible in Internal Medicine (AMA Board of Internal Medicine), or Family Medicine (AMA Board of Family Medicine), required.
  • Must successfully complete the credentialing process at the Hospital facility, required.
  • Basic Life Support (BLS), required.
Knowledge and Skills:
  • Proficiency with Epic or comparable enterprise EHR platforms, equipment and other healthcare and billing systems relevant to this position.
  • Strong desire to connect with community members and build deep relationships with our patients and their families.
  • Multilingual ability or cultural competency training aligned with the patient populations of eastern Brooklyn (Spanish, Haitian Creole, Bengali, or other languages spoken in the community), preferred.
  • Willingness to build expertise in value-based care, care navigation and population health management in a complex regulatory environment.
  • Interest in teaching and contributing to graduate medical education.
Physical Requirements
  • Position requires prolonged periods of standing, reaching, walking throughout the working day.
  • Position will be required to stoop, bend, lift, and carry items weighing up to 75 pounds.
