Responsibilities

• Care for Veterans in a team-based Patient Aligned Care Team model with strong nursing and ancillary support.
• Mostly outpatient continuity care: chronic disease management, preventive care, and same-day access for acute needs.
• Electronic medical records, embedded pharmacy, social work, mental health, and specialty referral support.

Compensation

• Competitive salary: $230,000 - $315,000 per year, based on training, experience, and formal VA pay-setting (Base Pay + Market Pay)

• Incentives: Recruitment/relocation and performance incentives may be available, subject to eligibility and facility needs
