Job summary

CenterWell Senior Primary Care — a Humana CenterWell Primary Care Organization unit — is seeking a full-time, on-site physician to provide comprehensive primary care to seniors in a value-based model. You will see 12-15 patients daily, manage complex cases, document promptly, and coordinate with a care team to achieve meaningful health outcomes.

You will hold an MD or DO, be board-certified/eligible in Family/Internal Medicine or Geriatrics, and possess an active SC license or ability to obtain.

Qualifications

• MD/DO with an active medical license in SC or ability to obtain
• 2+ years of clinical experience with senior populations
• Experience with Medicare Advantage or risk adjustment
• Experience supervising Advanced Practice Providers (NPs/PAs)
• Proficiency with electronic health records (EHR) and digital documentation

Responsibilities

• See 12-15 patients per day with longer, more in-depth visits
• Manage patients with complex and chronic conditions
• Complete accurate, same-day documentation to support patient care and outcomes
• Manage a daily clinical inbox (labs, messages, follow-ups)
• Coordinate care with specialists and care team members
• Apply Medicare risk adjustment principles through accurate condition documentation

This is a full-time, on-site, patient-facing role. You report to the Associate Medical Director and are responsible for both in-visit care and ongoing patient management. You will provide comprehensive primary care to senior patients within a value-based care model that emphasizes quality outcomes, preventive medicine, and long-term health management.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
