Job summary

Trinity Employment Specialists is seeking a compassionate Family Medicine or Internal Medicine physician for an established outpatient practice. This outpatient-only role offers a Monday–Friday schedule with no nights, no weekends, and no hospital call, along with longer appointment times and a collaborative care team focused on value-based care for older adults.

You will diagnose and treat acute and chronic illnesses, develop personalized treatment plans, and provide preventive care.

Qualifications

• Strong communication and leadership abilities.
• Experience in outpatient primary care preferred.
• Board certification or eligibility in Family or Internal Medicine and valid medical license.

Responsibilities

• Diagnose and treat acute and chronic illnesses.
• Manage conditions such as diabetes, hypertension, COPD, heart failure, and other chronic diseases.
• Develop personalized treatment plans and preventive care strategies.
• Perform annual wellness visits and preventive care.
• Collaborate with specialists and interdisciplinary care teams.
• Educate patients and families to promote healthier lifestyles.
• Maintain accurate EMR documentation.
• Supervise Advanced Practice Providers when appropriate.
