The Family Medicine Physician provides comprehensive, continuous, and patient-centered primary care, focusing on prevention, diagnosis, and management of acute and chronic conditions while coordinating care across the healthcare continuum.

Qualifications

• MD or DO from an accredited medical school.
• Completion of an accredited Family Medicine residency program.
• Board Certified or Board Eligible in Family Medicine.
• Current or eligible for an unrestricted medical license in the state of practice.
• Current DEA registration or eligibility to obtain.
• Strong clinical judgment and commitment to high-quality patient care.
• Excellent communication, interpersonal, and collaboration skills.

Responsibilities

• Provide comprehensive primary care, including patient evaluations, physical examinations, diagnosis, and treatment.
• Manage acute and chronic conditions while developing individualized care plans.
• Provide preventive care, wellness visits, screenings, and patient education.
• Order and interpret laboratory, imaging, and diagnostic testing.
• Prescribe medications and coordinate referrals and specialty care as needed.
• Collaborate with advanced practice providers, nurses, medical assistants, and other care team members.
• Maintain timely and accurate EHR documentation while supporting quality improvement and value-based care initiatives.

Position Summary
