Job summary

Lane County seeks a Physician Assistant to join its interdisciplinary primary care team at Charnelton Clinic, delivering comprehensive care within a patient-centered medical home model to a diverse community. The clinician will evaluate patients and provide education and referrals as part of a collaborative team.

Qualifications

• Bachelor's Degree or equivalent from an accredited college or university with major course work in primary care for physician assistants.
• One year of licensed Physician Assistant experience.
• State of Oregon PA license prior to appointment.
• Valid driver’s license at time of application and Oregon license by appointment if required to drive.
• Current BLS/CPR certification and required credentialing documents (DEA, NPI, license verification).

Responsibilities

• Provide professional, comprehensive primary medical care within a team-based patient-centered medical home.
• Diagnose and treat a wide range of health conditions; provide education and referrals as appropriate.
• Deliver care in a community health setting with interdisciplinary team collaboration.
