Qualifications

• NP practices to full extent of education, training and certification.
• Prescribes pharmacologic and non-pharmacologic interventions.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions.
• Develops and implements individualized plans to achieve population health outcomes.
• Demonstrates leadership and collaborates with Primary Care Team and interdisciplinary members.
