Eventus WholeHealth is seeking an Advanced Practice Clinician (NP/PA) to deliver comprehensive primary care within skilled nursing and senior living communities.

You will perform routine rounds, manage chronic conditions, and coordinate care with interdisciplinary teams and facility partners to enhance preventive care and outcomes for residents and families.

Competenze

• Current NP or PA licensure in the state of practice.
• Experience in geriatrics, family medicine, or internal medicine preferred.

Position Summary

As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will provide comprehensive primary care services within skilled nursing and senior living communities. This role includes routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and close collaboration with facility partners. Qualifications

• Current NP or PA licensure in the state of practice
• Previous professional patient care experience
• Experience in geriatrics, family medicine, or internal medicine preferred.
