Job summary

Eventus WholeHealth seeks an Advanced Practice Clinician (NP or PA) to provide primary care in skilled nursing and senior living communities in Virginia, with routine rounding, chronic disease management, and care coordination with interdisciplinary teams.

Must hold current VA NP/PA licensure, with geriatrics or family medicine experience, and demonstrate compassionate communication. This full-time role offers competitive benefits and no on-call.

Position Summary

This role includes routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and close collaboration with facility partners. Qualifications

• Current NP or PA licensure in the state of practice
• Previous professional patient care experience
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams
