Eventus WholeHealth is hiring an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to deliver comprehensive primary care within skilled nursing and senior living communities. You will perform routine rounds, manage chronic conditions, and coordinate care with interdisciplinary teams and facility partners.

The role emphasizes preventive care, improved outcomes, and a collaborative, value-based approach.

Formación

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff.
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams.
