Infinite Medical P.C (Managed by MedElite) invites applications for a Primary Care Nurse Practitioner or Physician Assistant to join our team serving residents in long-term care facilities. This is a part-time role in Cleburne, TX.

You will provide comprehensive care, coordinate with an interdisciplinary team, perform assessments, order tests, diagnose and treat, and help develop and adjust care plans. Licensure, DEA, and at least 6 months in long-term care are required; ability to lift 20

Qualifications

• Licensed Nurse Practitioner or Physician Assistant with current credentials.
• DEA license required.
• Minimum 6 months experience in long-term care.
• Ability to lift 20 pounds and stand/walk for up to 6–8 hours daily.

Responsibilities

• Provide comprehensive healthcare services and coordinate patient care.
• Work closely with a team of healthcare professionals.
• Assess, diagnose, and treat residents’ physical and mental well-being.
• Conduct thorough patient assessments, including medical history reviews and physical examinations.
• Order and interpret diagnostic tests.
• Develop accurate diagnoses and treatment plans.
• Prescribe medications and administer treatments.
• Provide counseling to residents and their families.
• Collaborate with the interdisciplinary team for seamless coordination of care.
• Manage chronic conditions and monitor progress.
• Make appropriate adjustments to care plans.
